In this role, you will:
- Provide chronic disease management for patients via telephone communication
- Contribute to the assessment of patients by collecting data via phone.
- Conduct CCM services for patients with chronic medical conditions to improve quality patient care and reduce hospital readmissions and unnecessary emergency room visits.
- Coordinate transitional care from an inpatient service to other settings as per the care plan, in collaboration with hospital coordinators (TCM)
- Assist with advocacy to clients and families for entitlements and other community services, including the New York Cancer Foundation and social work services here at NYCBS.
- Assist patients/families with disease treatment plans including self-care, symptom management, and available resources and follow-up care & and provide patients with clear instructions and education utilizing appropriate guidelines.
- Work closely with in-office providers to manage the day-to-day calls involving: symptom control, and medication management, and provide patient and family education.
- Educate the patient on properly contacting the physician's office for urgent care matters / or 911 when appropriate or unsure
- Gather patient clinical data to prepare the patient for a visit with a provider.
- Perform medication reconciliations, allergy verification, and review outside providers to ensure up-to-date medical records.
- Documents all patient communication clearly and concisely; gathers patient data and communicates timely/appropriately according to established guidelines
- Communicate with physicians, Nurse Practitioners, Social Workers, psychologists, or other health care providers regarding the patient’s status.
- Direct patients to keep follow-up appointments with the physician's office and to maintain self-home care based on physician direction and outcomes of nursing assessment.
- Meet and report all daily and weekly goals set by the Care Coordination Supervisor.
- Participate in all planned continuing education as necessary
We require: - Graduate from an accredited School of Nursing
- Current NY State license to practice as an RN
- Experience working with Electronic Health Record (EHR)
- Progressive proficiency with the utilization of available computer technology, including typing skills
- Customer service, leadership, communication, interpersonal and problem-solving skills
- Spanish preferred.
